The blood needs to have a certain blood pressure in order to achieve this, while there are also other systems that help.
In the instance of a mini heart attack, the heart can no longer pump blood at a high enough pressure. This means that it cannot pump the blood back up through the body as well as it should, causing it to pool at the bottom. This causes swollen ankles, a symptom that should be treated as an emergency if it comes on suddenly.
